The ‘shared psychosis’ between Trump and his followers: Yale psychiatrist

On Monday, President Donald Trump continued claiming full exoneration, even as more damning evidence was culled from the Mueller report.

And it appears to be exacerbating the President’s more extreme tendencies.

“Nobody disobeys my orders,” the President barked at reporters earlier today.

Raw Story spoke with Yale psychiatrist Dr. Bandy X. Lee, a renowned expert on violence and forensic psychiatrist at Yale School of Medicine. She has been a consultant to the World Health Organization since 2002 and is author of the authoritative textbook, “Violence: An Interdisciplinary Approach to Causes, Consequences, and Cures.” She is also the president of the World Mental Health Coalition and editor of the New York Times bestseller, “The Dangerous Case of Donald Trump: 37 Psychiatrists and Mental Health Experts Assess a President.” This public-service book was just released as an expanded edition alongside a major, interdisciplinary conference on presidential fitness that was held in Washington, DC (dangerouscase.org). She and her colleagues are currently preparing a mental health analysis of the Mueller report.

Raw Story: So, it seems that Robert Mueller’s report did not exactly clear the President of wrongdoing, as he’d previously claimed. So the President is lashing out. What do you think explains the President’s behavior?

I think we are reaching a pivotal moment here. Exactly two years ago, I organized a conference at Yale School of Medicine on how psychiatrists could ethically warn the public against the dangers of the current presidency and fulfill our duty to society. I invited some of the most preeminent psychiatrists of our day, including Drs. Robert Jay Lifton, Judith Herman, and James Gilligan. We put our analysis into the bestselling book, The Dangerous Case of Donald Trump, almost two years ago, as a public service, removing all conflicts of interest by donating all royalties. While Robert Mueller’s report may not have been enough to indict him, it is proving to be a wellspring of information on the president’s mental capacity, proving that our analysis was correct.

In our analysis, we stated that the president’s mental capacity was of grave concern and potential danger to society in his office by reasons of, among other things, impulsivity, recklessness, a lack of firm grip on reality, an inability to work with advisors, an absorption in self-interest that seemed to preclude considerations of national interest, a seeming inability to consider consequences before taking action, cognitive difficulties, paranoid reactions, an attraction to violence, and an absence of empathy that exceeded the normal range. The special counsel’s report now confirms the reasons why we believed the president’s condition is severe and would only grow worse over time in his current position.

Seldom do we have this quantity of detailed information on a person’s behavior, verified with the caliber of a criminal investigation of the highest order. Now, we can interpret the evidence with a high level of confidence, vastly exceeding what we ordinarily have for our own patients. We have thus formed a team to prepare a mental health interpretation of the special counsel’s report.

Raw Story: How do you think his followers are dealing with the fact that the report in fact contains many instances of ethical breaches by the President and his associates?

There were three ways in which we predicted, based on probability, that his presidency would be dangerous.

First, we said that he was more dangerous than the public suspected, that he would not “pivot” to become more presidential as many expected two years ago, but that the verbal abuse, abusive treatment of women, incitement of violence, endorsement of violence in public speeches, and the taunting and alienation of allies and enemies alike turned out to be only the tip of the iceberg.

Second, we said he would grow more dangerous with time, as his psychological dangerousness translated into social, cultural, and geopolitical dangerousness with the power of his position. His proneness for cruelty has created thousands of young orphans and bereaved families and left entire segments of the population without relief from climate-related disasters; his affinity for despots around the world has encouraged human rights abuses and murder of journalists; and his attraction to violence has encouraged a culture of widespread hate crimes, white supremacist and religious terrorism, heightened risk of war in unstable regions, and a renewed nuclear arms race.

And third, we said that he would become increasingly uncontainable because his followers would, in a sense, take on more of his symptoms and grow “sicker” with time. Therefore, the ethical breaches by the president and his associates will not matter, just as facts or evidence will not matter, since his supporters are now largely disconnected from reality, the way the president is disconnected from reality. We see this in his low-income supporters whose taxes are going up while reports of wild windfalls for corporations and the super-wealthy are resulting from the president’s tax policy, that they still believe the president is fighting for “the little guy.” We see from farmers who are losing their livelihoods because of the president’s trade policies, or people who are at risk of dying from his proposed healthcare repeal, that no amount of proof will turn them from their fixed loyalties.

In psychiatry, we call this separation from reality “psychosis,” and we are witnessing a “shared psychosis” with the president. So it is no wonder that his followers will deal with the new information in all kinds of maladaptive ways, for example through displacement (“The investigators need to be investigated”), projection (“This is the work of angry partisans”), minimization (“Anyone would do what the president did”), or denial (“It was total and complete exoneration”). The ability of his followers to go to their graveyards—and bring the nation down with them—believing what they do demonstrates the power of the mind and the unique danger of allowing a president with mental pathology to stay on.

Raw Story: Is there hope?

While his followers seem recalcitrant, they can also suddenly flip when they complete their idealization-devaluation cycle. In other words, once they are disappointed in their leader, they could turn on him drastically. Much of this depends on removing Donald Trump from power—what will matter is not the evidence or reasons for, say, impeachment, but the fact that he is impeached or removed from power, regardless of the reasons. Once the removal takes place, the worshipping will likely stop.

This is why we continue to press on the situation of medical need that we see, regardless of politics, since mental health professionals base their assessment on facts, evidence, clinical observations, and science. The whole nation needs better grounding in reality, which is where further investigations or impeachment proceedings could be helpful for the sake of the nation’s mental health, regardless of political consequences. Of course, we need to be mindful of the greater dangers that would accompany this process, especially if the president’s powers continue unchecked.

‘Reckless, impulsive, self-destructive’: Psychiatrists warn Mueller report gives more evidence Trump is mentally unfit

Three psychiatrists have written an editorial for the Boston Globe warning that special counsel Robert Mueller’s report on Russian interference in the 2016 presidential election shows more evidence that President Donald Trump is mentally unfit to hold office.

The psychiatrists — Dr. Bandy X. Lee of Yale, Dr. Leonard L. Glass of Harvard Medical School, and Edwin B. Fisher of University of North Carolina-Chapel Hill — argue that Mueller’s report provides a disturbing new window into the president’s behavior, which is frequently marked by impulsive emotional meltdowns.

“The pattern that emerges of the president is one of rash, short-sighted decision-making, without consideration of consequences,” they write. “Reckless, impulsive moves that are self-destructive, despite the intention of self-protection, are characteristic of dangerous impairment. They impede Trump’s capacity to prioritize national security.”

They then quote from sections of the Mueller report that illustrate these behaviors, including his efforts to dangle pardons to potential witnesses and his effort to get Mueller fired as special counsel.

They say that his ordering former White House counsel Don McGahn to get rid of Mueller should be seen as especially alarming given how inherently self-destructive it was.

“In a post-Nixon era, and especially after the experience of firing Comey, a rational, non-impulsive person with reality-based decision-making would hesitate before pursuing this path,” they write. “He apparently believed he could use all the powers at his disposal to have his way, and almost delusionally expected impunity. Such a mindset of false beliefs in freedom from consequences is extremely dangerous when coupled with power and is great cause for alarm in the US presidency.”

Top psychiatrists reveal how Trump’s mental incapacity was exposed by Mueller

On Tuesday, former FBI director Christopher Wray told Congress that he would not characterize the FBI probe into the Trump campaign as “spying.”

Attorney General William Barr stirred up controversy when he used the fraught term to describe the agency’s investigation, sparked by information that Trump associates were compromised by foreign powers.

President Donald Trump has long maintained that he’s the victim of various conspiracies to undermine the administration. And he’s often said so, causing alarm that his followers might resort to violence.

Cesar A. Sayoc Jr. sent bomb threats to media outlets and public figures perceived to be Trump’s enemies. The President’s paranoia, evident in non-stop Twitter blasts and aired at rallies, has caused mental health professionals to question his fitness for office.

Raw Story spoke with Yale psychiatrist Bandy X. Lee about what prompted her — and other mental health specialists — to speak out about the president’s psychological problems, despite the so-called Goldwater rule that prohibits psychiatrists to diagnose public figures.

Lee is a renowned expert on violence and forensic psychiatrist at Yale School of Medicine. She has been a consultant to the World Health Organization since 2002 and is author of the authoritative textbook, “Violence: An Interdisciplinary Approach to Causes, Consequences, and Cures.” She has done numerous forensic reports for criminal and civil cases, as well as on institutional analyses, most notably of New York City’s notorious Rikers Island jail complex, which helped to initiate reforms.

She is also president of the World Mental Health Coalition and editor of the New York Times bestseller, “The Dangerous Case of Donald Trump: 37 Psychiatrists and Mental Health Experts Assess a President.” She and several co-authors have submitted a mental health analysis of the Mueller report to the people via the press (Huffington Post covered it) and to Jerrold Nadler, chairman of the House Judiciary Committee. The report (currently posted here) will be highlighted and hosted by the Boston Globe later this week for wider dissemination.

Raw Story: You did a “report on the Special counsel’s report,” which has a serious title but then a provocative subtitle: “If One is Too Incompetent to Commit a Crime, despite Trying Hard, is One Competent to be President?” How did you come to do this report, and what does it cover?

Lee: The special counsel’s report was replete with information for a mental health interpretation. Seldom do we have such voluminous information on a person’s behavior, verified at the level of sworn testimony. Hence, my colleagues and I saw this as a part of our ongoing, primary professional responsibility to protect society and to warn against potential harm to public health, as our ethics and the law require. The report is an attempt to continue educating the public.

The special counsel’s report could not be more informative about mental capacity, which is what the report addresses. Mental capacity is the ability to make sound, rational, reality-based decisions free of impulsivity, recklessness, paranoia, and false beliefs.

Since it is about function, not diagnosis, it has less to do with what specific illnesses are affecting someone. Rather, it has to do with whether one possesses the ability to do one’s job. A person can have a severe mental illness, for example, but be receiving proper treatment and function perfectly well—the presence of mental illness really says nothing about job performance. On the other hand, someone may not have a mental illness but still be incompetent and lacking in the basic capacities to do a job.

For a functional exam, a personal interview is not as important as observations by the person’s coworkers and colleagues of one’s actual performance, and this is why the special counsel’s report is so valuable. It is full of detailed and relevant information through the direct testimony of multiple allies and opponents, which allows us to make an appraisal with a level of confidence that far exceeds anything we can hope for in our ordinary practice.

Raw Story: Who wrote the report?

Lee: We are all authors of the public-service book, The Dangerous Case of Donald Trump, which came out of what we saw as a medical need, and was one of our first efforts to try to fulfill our societal responsibility. The fact that the book predicted much of the course of the current presidency and has received high acclaim among our peers and the public should give us credibility.

Writers of the report included myself; Dr. Edwin Fisher, clinical psychologist and professor in the Department of Health Behavior at the Gillings School of Global Public Health of the University of North Carolina-Chapel Hill; Dr. Leonard Glass, psychiatrist, associate clinical professor at Harvard Medical School, and senior attending at McLean Hospital; Dr. James Merikangas, forensic neuropsychiatrist, clinical professor at George Washington University School of Medicine and Health Sciences, and research consultant for the National Institute of Mental Health; and Dr. James Gilligan, forensic psychiatrist, adjunct professor at New York University School of Law, and consultant to the Violence Prevention Alliance of the World Health Organization. All are of the highest caliber, if not at the very top of their fields.

Raw Story: What does the report say?

Lee: Our report is quite damning. What the special counsel’s report revealed, through consistent and abundant data, was a pervasive and profound pattern of lack of capacity.

This was demonstrated by: lack of basic comprehension (or the ability to take in information and advice without undue influence from false beliefs or emotional need); faulty information processing (or the ability to appreciate and make flexible use of information and advice without false representation); lack of sound decision making (or the ability to consider consequences based on rational, reality-based, and reliable thinking without interference from impulsivity, false beliefs, or fluctuating consistency); and behavior that places oneself or others in danger (such as inciting one’s followers to commit acts of violence and boasting of one’s own repeated violence). These are crucial failures in the basic components of mental capacity test, which in his position constitute a medical emergency that requires a response.

Raw Story: Is the Goldwater rule relevant here?

Lee: There are many misinterpretations of the Goldwater rule, and it is important that we make clear: the Goldwater rule is a call to action, for psychiatrists to fulfill their primary responsibility to society by participating in activities that improve the community and better public health.

Our obligation is not to a public figure but to society, and the rule states that, when asked about a public figure, we educate the public in general terms while refraining from diagnosis (or the equivalent). We adhere to the Goldwater rule by refraining from any diagnosis and, more importantly, uphold its principle by acting for the benefit of society and by doing what we can to protect its health and wellbeing.

Raw Story: What can health professionals do?

Lee: We will continue to use our skills as expert witnesses to educate the public so that it can lean on its representatives to act. They are the ultimate authorities now, and we are doing our ethical and legal duty to report danger. As for now, given the serious concerns we and others have raised, we have recommended that the president agree to undergo a formal evaluation by an independent, nongovernmental panel of experts. Neither the president nor his associates have documented a valid mental health evaluation. With the preponderance of evidence and the extreme dangers implied in presidential incapacity, it is critical that he clarify this issue for the good of the country.

We at least have to make a reasonable attempt to obtain a personal interview. We have recommended that he agree to one within three weeks. Since this is a functional, not diagnostic, assessment, an interview is not as central or even necessarily informative as observations of performance by colleagues and associates. Therefore, in the event that he refuses, we believe that we will still have sufficient evidence to come to a conclusion that the president lacks the fundamental mental capacity to discharge the duties of his office. It may become necessary that we move ahead with that conclusion.

Trump’s 10,000-plus lies are not just random bluster – they are an attempt to remake reality as he wants to see it

In Part 1, we looked at Trump’s lies through the lens of the Washington Post’s own “bottomless Pinocchio” lists as well as three academic studies based on their data, before turning to a broader perspective. That involves seeing Trump’s lies as part of a larger rhetorical strategy, as described by Jennifer Mercieca, a historian of rhetoric, whose book, “Demagogue for President: The Rhetorical Genius of Donald Trump,” [table of contents here] will be published next year.

The bottom line of Part 1 is that Trump’s lies can be understood as expressing his view of the world — how he would like things to be, for whatever purposes he may have, some quite enduring (as seen in his “bottomless Pinocchios”), others perhaps spur-of-the-moment. As Mercieca argued here, the Mueller report can be read as further confirmation of Trump’s authoritarian orientation in word and deed, in sharp contrast to the “total exoneration” he falsely claims it to be. This is entirely typical. Demagogues and tyrants have always used lies this way. But they cannot and do not act in a vacuum. The broader context in which they operate is the focus of Part 2.

Trump’s team and Trump’s voters

Trump doesn’t act alone. “Trump’s lies are supported by an expert propaganda team,” Mercieca said. “Propaganda seeks to shape understanding by subverting rational thinking. Trump’s propaganda team amplifies his lies (see also [James] Comey’s op-ed about how Trump makes everyone accomplices in his lies), but they also provide him with strategically useful talking points and arguments.”

With his signature attacks on “political correctness,” Trump tries to claim the mantle of a truth-teller — though perhaps without being taken literally. But that’s a problematic claim, to put it mildly.

“In ancient Greece a ‘truth teller’ was known as a parrhesiastes; such a person was a ‘fearless’ speaker, they spoke truth to power, endangering themselves,” Mercieca explained. “What they said was believed to be true because of the risk that they took when they said their unpopular truths. Trump doesn’t speak truth to power, he doesn’t risk his life with his unpopular truths. Trump isn’t a truth teller in the classical sense.”

Nonetheless, “Trump’s lies are popular with his base,” Mercieca acknowledged. She suggests this passage from Hannah Arendt’s “Origins of Totalitarianism” can help:

A mixture of gullibility and cynicism had been an outstanding characteristic of mob mentality before it became an everyday phenomenon of the masses. In an ever-changing, incomprehensible world the masses had reached the point where they would, at the same time, believe everything and nothing, think that everything was possible and that nothing was true … Mass propaganda discovered that its audience was ready at all times to believe the worst, no matter how absurd, and did not particularly object to being deceived because it held every statement to be a lie anyhow.

“I think gullibility and cynicism are a good explanation for Trump voters,” Mercieca said. “They think everything is a lie anyway, so why not cheer for their guy who lies better than the rest of them?”

Three legs of tyranny and how they shape lies

But there’s another way of viewing the pathologies of tyranny, described by Elizabeth Mika, a contributor to “The Dangerous Case of Donald Trump,” which I reviewed here in October 2017. Her contribution, “Who Goes Trump? Tyranny as a Triumph of Narcissism,” was focused on how the threat of this demagogic malignant narcissist interacts with related narcissistic tendencies found among his supporters and society at large. Lies are omnipresent in the world she describes, but the tyrant’s lies — expressing the world-as-he-wants-it and imposing it on everyone around him — can only succeed as part of a broader dynamic of mendacity.

“Tyrannies are three-legged beasts,” Mika explained: There’s the tyrant, his supporters and the society as a whole, and they are bound together in different ways, always with some profound need for deception. The nature of lies that a tyrant or would-be tyrant tells are reflections of all three, not just the tyrant alone. But the idea that Trump would become “normal” was never in the cards, as she reiterated when I reached out to her.

“The dismantling of American democracy under the leadership (if one can call it that) of Donald Trump is proceeding as predicted, so much so that there is not much to add beyond the somewhat bitter ‘We told you so,’” Mika said. “It is just that the punditocracy is perhaps finally catching up.”

The explosive increase in Trump’s lying noted by the Washington Post was no surprise to Mika. She pointed back to something she wrote just after Trump’s inauguration:

What we know about malignant narcissists is that they psychologically decompensate once they achieve the ultimate position of power. They worsen in every possible way: become more grandiose and paranoid, more aggressive and demanding, and progressively less in touch with reality (and Trump has never been fully in touch with it).

We can expect his narcissistic rage to intensify in proportion to his growing grandiosity and paranoia ….

There has never been a case of a malignant narcissist in power whose pathology improved, or even remained stable: they always deteriorate, and often rapidly, as they become drunk on (what they see as) now unlimited power and adulation.

“I wrote that in early 2017, and other pieces in a similar vein all throughout 2016. So none of it is surprising,” Mika told me. “What’s fascinating, in a grim way — though also not surprising — is the blindness of our press, et al., to these developments,” which brings us back to the media’s inability to report what’s right in front of their eyes: an avalanche of lies.

This isn’t on Trump. It’s rooted in our own elitist narcissism, Mika argues:

I came up with the term “narcissistic blindness” to capture this phenomenon rooted in a seeming conviction that It can’t happen here because we are the beacon of democracy, we have our esteemed institutions, and other self-serving fairy tales reasserting American superiority and invincibility to Americans — as if we were exempt from the universal laws governing humanity since time immemorial. This narcissistic delusion of our exceptionalism is our downfall.

That describes the third leg of tyranny. The one so deep in denial that it thinks it’s not even there. The one that afflicts the media itself directly, and that plays such a big role in rendering it incapable of dealing with Trump’s lies.

As for the other two legs, Mika said:

Another term applicable here is “narcissistic collusion,” which describes the bond between a leader with Trump’s character defect and his sycophants and followers. They mirror their imagined greatness to each other, and this mutually reinforced delusion of grandeur becomes an unshakable bond, superseding any other considerations, like respect for truth and decency, or even a concern for one’s reputation; Comey’s recent New York Times piece touches upon it, but does not go into the narcissism and its spectacular bonding properties.

Both Trump’s close followers and his mass base get swept up in such delusions. The latter are driven by a need for revenge, Mika argued in “Who Goes Trump?”:

… for the tyrant is always chosen to perform this psychically restorative function: to avenge the humiliations (narcissistic wounds) of his followers and punish those who inflicted them.

But the followers are profoundly confused about the actual sources of their wounds. Their misperception “is based on the displacement and projection characteristic of the scapegoating process that becomes an inextricable part of the narcissistic collusion between the tyrant and his followers.”

In turn the displacement, projection and scapegoating are reflected in some of Trump’s lies listed above: those around immigration, most obviously, but also those that overstate the impact of trade deficits, those that exaggerate military spending, and — to the extent the followers fuse their identities with Trump’s — his lies about he himself being persecuted. The revenge-driven power of narcissistic collusion makes such lies especially impervious to rational refutation. Conventional ways to try to understand such lies will inevitably fall short.

“The sycophants and followers of a leader who fulfills their narcissistic fantasies will do anything to support him, up to and including self-destruction,” Mika said. “No lie is too big or outlandish when this deeply irrational, even psychotic process is engaged. So we can expect 10,000 more lies from Trump, served more brazenly now that he feels emboldened by putting Mueller’s investigation behind him.”

Of course, it may not be that easy. But Democrats’ refusal to let Mueller’s investigation vanish will only enrage Trump further, leading to even more lies.

Tyrants do not arise in a vacuum just as tyranny does not spring on the world unannounced. It takes years of cultivation of special conditions in a society for a tyranny to take over. Those conditions invariably include a growing and unbearably oppressive economic and social inequality ignored by the elites who benefit from it, at least for a time; fear, moral confusion, and chaos that come from that deepening inequality and a breakdown of social norms, and growing disregard for the humanity of a large portion of the population, as well as for higher values.

The positive vision that Democratic presidential candidates are striving to articulate needs to deal effectively with this reality. If it doesn’t, even defeating Trump in 2020 won’t be enough. If the conditions that raised him to power continue, there will be another much like him to come — perhaps a more intelligent, more effective one. Trump’s lies will continue — whether from his lips, or from those of others — as long as we fail to confront the truth about ourselves as a society.

The reason the US has not been more vulnerable to our enemies and adversaries since World War II is because they whole world has figured out Trump's forms of mental illness and know how to read and manipulate him. Things like his psychological projection which is more than just a lie. The president reacted by citing a Fox News poll showing “58 percent of people say that the FBI broke the law” by investigating him, and calling for investigators to be jailed for “treason.”
